Oregon man has died after falling into a meat blender, An Oregon man has died after falling into a meat blender according to an April 30 report from Fox News.

The victim, 41-year-old Hugo Avalos-Chanon of Portland Oregon, was reportedly cleaning a meat blender inside a meat-processing plant when he fell in, and was killed.

Fox News reports that the paramedics and sheriff's deputies were called around 11:45p.m. on Friday after another employee saw Avalos-Chanon fall into the machine. That employee hit the emergency stop button, but it was too late. When authorities arrived they found the 41-year-old cleaning contractor dead inside the blender.

Firefighters returned to the scene on Saturday to take the machine apart so they could remove Avalos-Chanon's body.

The state medical examiner, said that Avalos-Chanon died from “blunt-force injuries and chopping wounds.”

The Occupational Safety and Health Administration (OSHA ) reported that the machines weren't locking properly last fall, but there is no word if that is what caused the accident that resulted in Avalos-Chanon's death.
